About Patient Safety Net

Patient Safety Net is a simple and direct way for staff to report concerns about patient safety that haven’t been resolved through other pathways.

A patient safety concern is anything that causes or may cause harm to patients. It can be physical or psychological.

Patient safety concerns can be reported in person, or by email, phone or through a secure portal. Patient safety stewards at each hospital and health service provide a safe space for staff to raise and discuss their concerns.

Patient Safety Net complements existing patient safety reporting processes and doesn’t replace them. It’s not for urgent patient safety concerns, or if a patient’s condition is getting worse or not improving as well as expected.

Using Patient Safety Net

Patient Safety Net can be used by anyone who works for Queensland Health, including contractors, agency staff, students and volunteers.

It was trialled in 4 hospital and health services and it’s being rolled out across Queensland Health and Queensland Ambulance Service in 2025.

We’ve also introduced a staff program called High Value Conversations, to improve communication about patient safety. It’s available to all hospital and health services and Queensland Ambulance Service.
